The warning was seen. Documented. Discussed. And ignored.
On January 28th, 1986, the Space Shuttle Challenger sat on the launch pad in
freezing weather. Ice coated the structure. Temperatures were the coldest the shuttle had ever
faced. Inside its solid rocket boosters were rubber seals called O-rings.
Their job was simple. Seal the joints. Stop hot gases from escaping during ignition.
But engineers had been worried about these O-rings for years. On earlier flights,
they had already seen damage. Burn marks. Erosion. Signs that hot gas
